Parents of students in grades 7-12 in the Boise School District are requested to participate in the annual Student Wellness Survey. To allow their child to take the survey, parents must complete an Opt-In consent form.

The survey aims to gather important information on student mental health and well-being, which assists schools in addressing the needs of their students.

If parents encounter issues with the Infinite Campus Parent Portal, an alternative Opt-in Form can be filled out online at this link: [Opt-in Form](https://bit.ly/406CDud).
